New Patient Registration Form

Please go to your computer and click on the new patient PDF file to print Dr. Suh's new patient registration forms and forward to my secretary by one of the following methods:

1. fax completed forms to (425) 738-8366

2. scan and email to receptionist@eastsidepsychiatric.com

3. send to 2820 Northup Way, Suite 105, Bellevue, WA 98004.

Please also complete and send the screening tools below for some of the most common conditions treated in this office.

Once approved and cleared for an appointment, please click on Current Patient tab on the main page for instructions on how to set up the telemedicine session. Until the Covid pandemic is mostly over, Dr. Suh will been seeing his patients virtually only. We will provide updates when he will begin seeing some patients face to face.
